
Insurance & Appointment Information
We work with most health insurances, including:
Blue Cross Blue Shield, Empire New York State - NYSHIP, Federal Blue Cross Blue Shield, Aetna, American Specialty Health, 1199 Health Fund, GHI, United Healthcare, Oxford, "No-Fault"(Auto Accidents)
and Workers Compensation.
​We do accept HSA and FSA cards as payment
and can provide receipts for reimbursement as needed.
If you do not see your insurance listed one of our staff members can verify your insurance benefits for you.

Appointment Policy
At House of Health, our main job is providing the best care for our patients. We ask that you provide at least 24 hours' notice if you need to cancel or reschedule your appointment.
Appointments that are canceled, rescheduled, or missed with less than 24 hours' notice will be charged 100% of the scheduled session fee. This fee is the patient's responsibility and is not billable to insurance.
We understand that true emergencies and unforeseen circumstances can occur. In those situations, exceptions may be made at the discretion of the practice. Thank you for your understanding and cooperation.
