
Insurance & Appointment Information
At House of Health, our main job is providing the best care for our patients. We ask that you provide at least 24 hours' notice if you need to cancel or reschedule your appointment.
Appointments that are canceled, rescheduled, or missed with less than 24 hours' notice will be charged 100% of the scheduled session fee. This fee is the patient's responsibility and is not billable to insurance.
We understand that true emergencies and unforeseen circumstances can occur. In those situations, exceptions may be made at the discretion of the practice. Thank you for your understanding and cooperation.
Updated Insurance Information
Please provide our front desk staff with updated insurance information for the 2026 calendar year.
As a reminder we do accept HSA and FSA cards as payment
and can provide receipts for reimbursement as needed.

NYSHIP Insurance Updates
Please be aware of your NYSHIP insurance benefits and how they can be utilized at our office. Chiropractors obtain authorization and office visits based on pre-certification. Additional visits can be requested when medical necessity can be supported with a documented injury or spinal complaint. Unlimited chiropractic visits for the year.
Acupuncture (Out of Network) and Medical Massage (Out of Network) obtain authorization and office visits based on documentation review of medical necessity. Although we strongly believe maintenance is the key to preventing injuries, insurance based care does not cover maintenance visits.
Massage therapy visits cannot be used for "convenience or comfort" and need to be documented with a set treatment plan and referral in place. Relaxation, stress related massage or "preventative care" will not be covered by your plan.
Starting January 2026 Massage Therapy and Acupuncture are approved for 20 sessions each- per year. The 2027 benefits for these services have not been set, but appears to be cut drastically compared to prior years. We will keep our patient base posted as new information comes in and insurance benefits are confirmed for 2027.
